Community Care for Alcohol & Other Substance Use in Medical Settings
Co-care is a registry designed to assist primary care clinicians in integrating care for participants with unhealthy substance use into routine medical care. As behavioral health care is increasingly integrated into medical settings, especially primary care, the focus is often on mental health.
Primary Objective
Examine the efficacy of Co-Care, in comparison to EUC (Enhanced Usual Care), for reducing days of opioid, stimulant, and heavy alcohol use.

Hypothesis
Participants in the Co-Care condition, in comparison to those in EUC, will have reduced total days of use in Months 4-6.
